Old Evaluate Sheet

Hey, there are a bunch of old gems floating around, and no one seems to have the old evaluate item sheet. I've heard a couple people say that they have one, or had one, or something, but never have it with them at the time. Can someone scan it if they've got one and make sure that there are plenty of copies of both the new one and the old one at logistics?

Actually, now that I think about it, it would probably be a good idea to just cash out all of the old gems at logistics, that way people don't need two sheets. (I'd still like an old sheet anyways, if someone's got one, I'd appreciate an email so we can use this build.) Flush the system of them and whatnot.

I have two different sheets so I think one of them is the "old" sheet. When I get home I will double check, scan them, and e-mail them if need be.

I have a copy of the old sheet and the new one, and both have been given to staff at the last staff meeting. So they should already have copys. If for some reason they dont, I still have the old one on the desk I am sitting at as I write this so it should be getable. (Not easy to scan however as it has some wear and tear damage since I carryed it for a few years.)
