Unfortunately, NPCs don't have the market cornered on these things. In the case of PCs, the same factors tend to combine to prevent resurrections.

In my experience three things account for how likely a given group of PCs is to resurrect more than anything else: 1. How well the players are prepared for the encounter 2. How well plot takes variables into account when scaling the encounter 3. How closely the marshals are watching 1 and 2...
